I think I will be able to give you a some open source solution instead paid third party like Telerik. So, I will wait for your document to verify and to give you a suggestion. I am very much confident on this matter. Queries: - IS this form will be filled by anonymous users? I mean is there will be any login form? - Your application seems to be multiple user application where role management is important. Can you please clear my doubt? - What should need to do after feel the form? Should this need to display anywhere or any CRUD operation is required or not? - Seperate tables? - I am assuming form has entities and we need to save according to entities, is this correct? I have extensive experience converting PDF/Word documents into an online user interface. Additionally, I have experience with MVC/Bootstrap creating mobile adaptive pages. You mention Telerik; their Kendo UI product is my third party library of choice. I assume the quoted budget range is per form, as these can range in complexity depending on the specific requirements. Given my skills and years in the industry, I would ask for the full $250 per form.
