Access Database Conversion to Web Application

Convert Access Database to Web Application using MySQL as backend database. Project will involve converting existing Access forms to HTML forms / style layout, conversion of Access reports to reports that will be displayed via HTML, conversion of Access data entry forms to HTML that will be added to MySQL database. Initial portion of project will be to convert all Access tables to MySQL tables, convert queries to MySQL queries, reports, forms, etc. Desired end of project is that all previous Access database routines are performed using a web application. Top 5 items will be discussed once programmer is selected. Additionally, audit logging will be added so that if a client record is viewed or modified there is a record of who did it.
