Axapta 3.0 Upgrade / Migration - repost

I have an Axapta 3.0 server with an SQL 2000 instance on the same windows 2003 32 Bit server which is running out of resources. the Axapta application has also bee heavily customized. I have the following requirements; * move the SQL database to a SQL 2005 or later server (depending on compatibility with Axapta 3.0) * move the Axapta client to a new RDS Server. * upon review of the application, discuss options for upgrading the entire platform to Dynamics AX 2012 R2 or another possible option.
