Create small database with Reporting functionality

The program will consist of an Access database or other database program, using a switchboard and forms to input data. We also would like to generate two types of reports, one will be a vector art certificate, dynamically pre-filling information about the customer (see attached file), and the other will consist of the company along with the HD Make/Model/Serial. Both should print to a PDF file. ***Switchboard*** The switchboard will consist of at least 3 fields: • Create new/ Edit existing Customer/Hard Drive Scanning session 1. We need to collect the Company name, contact name, city, state, zip, phone, email. We will need to record the make/model/serial of the hard drive under the company. If necessary we would like to go back and edit existing data. • Reports – we want to print reports to PDF. We will split them into two categories: 1. Report based on data recorded while scanning the HD Serial #s. A menu should pop up offering a print by date option/print all if multiple job sessions are recorded in the database. Printout Certificate of Destruction & Recycling – some data is static while other fields are dynamic based on info typed in during hard drive scanning session. A menu should pop up offering a print by date option/print all if multiple wipe sessions are recorded in the database (same as the 1st report). If you look at the image, the red information will be generated from what we type into the database. Exit – Exit the program I will supply the vector image of the certificate
