Form to be updated

I need a form to be updated as below: Basically the form is a ‘Player Transfers Form’; where user enters Ref No and the rest of the data fills automatically from a one list of players and I want the followings: 1. The user should be able to use two Players' lists (called Players' Register) in the same time for limited period usually 2 weeks. Afterwards will have only one option (using the latest Players' Register, which is amended by the admin. Basically the user once he enters the number in the field Using Players' Register Issue No: as per attachment Players' Register 1; user will have the option to use the players only from that list. 2. Two fields should be added below the form for the total value of the players to be transferred (Fields A and B above) as Players' Register 2 attached: • The first field would be the totals of the players transferred out. That is fields 1 - 3 - 5 - 7 -9 on the diagram above. • The second field would be the value of the players transferred in. That is fields 2 - 4 - 6 - 8 - 10 pn the diagram above.
