Custom Search Bar: WP Site + External

We are slowly building a web site for our library. We have a basic HTML form that searches our library catalog on another web site. We also need to retain the search-the-page function in WordPress. Combining these two into one search bar would be more functional with less clutter, save space, etc.
Cleveland Public Library has a beautiful example of what we'd like done on [login to view URL] top-right header, done on DNN/ASP.NET. One search field with radio buttons to select between the different searches.
We're also a CLEVNET library, so we use the same catalog system. Basically I just want that ripped off directly, made into PHP, very basic .css (just enough to work, we can make it pretty...) and stuck in our WP template. (WPZoom's "Horizon" - to replace the header banner ad area.)
