Code a calculator widget for Wordpress site - performing a simple Index Match (excel) style formula to data set (gender, age, decade = years)

I need a simple calculator widget coded to be placed on a Wordpress site inside a Visual Composer block (so a mix of HTML, CSS, JS? but open to your suggestions as I am not a technical developer).
The user chooses if they are male/female, chooses an age (from a predefined list: 0, 1, 15, 25, 45, 65, 85, 96) and chooses a decade (1880-2010s). Instantly they are presented with the life expectancy from the dataset.
I have it working in Excel using an Index Match for decade/age (two different sheets for male/female). E.g. if you were a [MALE] aged [45] in the decade starting [1960] you were expected to live for [72] years.

Also see visual design (open to suggestions) and just needs to scale width responsively. Mockup done in Sketch so could give Zeplin if required?
