Urgent Custom Page Contact Form Overhaul

I'm seeking an experienced Web Designer to quickly remove a non-functioning form from a custom page on my website, and replace it with a new, functioning contact form. This form should connect directly to my email. The fields should include name, email, wear date, how did you hear about us, describe your ideal dress and the aspects you desire from your inspiration. There needs to be a way for the customer to upload pictures which will come along with their contact message. There also needs to be a "thank you for contacting us" page once the order form has been submitted.
The new contact form should be:
* Located on my "Custom" page.
* Designed in a clean and minimalist style.
Your skills and experience should ideally include:
* Expertise in web design and form creation.
* Proven experience fixing and creating email contact forms.
* Excellent aesthetic eye to create a clean and minimalist design.
* Ability to work quickly and efficiently, as this task is urgent.
