Take 20 inputs from a customer & fill out a PDF form with several pages

1. There are 20 data inputs that have to be taken
2. Several of the data inputs are to be filled at several points on the PDF file
3. PDF file has several pages (50+) but the ones that have to be filled are <10
4. Saved PDF has to be stored on the server and emailed to some people
5. Who to email to has to be stored in the DB
You have to design the UI form to take the input, store in a table and the PDF output.
Experience with filling out PDF documents in code is a must. Please send me some examples of your work in this space if you are serious about the project.
