PHP Large File Upload to LogicalDoc Server

We have an internal LogicalDoc Server and need the ability to upload large files to it through our internal website.
It is a community edition logicaldoc server and is on an internal network not visible to the outside world.
This will be sewn into a pre-existing web app. It would be best if it came with an [login to view URL] to show an example of calling the uploader.
Requirements:
Use Latest Bootstrap
PHP 5.5 compatible
Must have multi file select
Must have drag and drop capability (may be on different tab or however works best from and end user standpoint)
Must be able to upload files up to 500MB
Must upload to a pre-selected folder
Must show upload progress
Can use LogicalDoc SOAP, MTOM, or RESTful services whatever is fastest and easiest
Must have clear variables for server name, port, folderId, userid, and password
Can be it's own page that receives post information from a form (e.g. folder ID and folder name)
It would be best if you could test on your own LogicalDoc server first. I apologize but this server cannot be exposed to the outside for testing.
