Magento Checkout Order Review Step1

I'd like to install a check box on the order review step, which has a sentence next to it which says "Is your business/institution VAT exempt?". This check box should only appear for customers who are billed in the United Kingdom. If the customer UNTICKS the box, a 20% VAT will be applied to the products on the order, and VAT of 20% issued on invoice.
