Updates to Wordpress site - custom PHP coding required

We have 3 changes needed for our Wordpress site. We are using Gravity Forms to allow people to register for our sports league and have added some custom code to push the submissions to our customer management system (CMS). The changes relate to this and how some data is pushed. All of the custom code is in our [login to view URL] file.
Project requirements:
1. Add error log and email
- If there is an error when adding/updating a player or contact log the error and send an email
- If there is an error the API returns a response like this:
"{"data":{"addEntityValue":null},"errors":[{"message":"Choice Adult 2XL not found for choice list Jersey Size (Players)","locations":[{"line":1,"column":11}],"path":["addEntityValue"]}]}"
- Place that entire string in the email and error log.
2. Push the data for "Team Code" to our CMS
- We added a new field called Team Code to our forms. Need to push this value to the CMS with the other fields.
- The field data should be pushed to both 'Contacts' and 'Players' in our CMS
- Effects these two forms: [login to view URL] and [login to view URL]
3. The password reset form is not working
- This is not related to the Gravity Form
- When a user goes to: Login > Forgot Your Password. The submit button does nothing.
