One Page to be Inserted into Inspire Wordpress Theme

I require some information to be inserted into an existing Wordpress website.
I need the following added to the front/home page:
Some text (which I will supply)
Add a video (I have the embedded code ready to go)
Add a form with only 2 fields ... Name and eMail address, plus a submit button
Once the user clicks on the submit button they will be given access to a PDF file (which I already have)
.... maybe the user needs to be redirected to another page to obtain the link to download the PDF file, but you can discuss that with me.
I am after something very basic similar to the following website: (take out the "_" in the URL).
http://more_clients (dot) com (dot) au
Work must be W3C compliant, and integrate seamlessly into the Wordpress site. The Wordpress theme is called Inspire.
