Figure out why options box no longer shows up on page (& repair)

My web site is [url removed, login to view], which features deals of the day from various sites. I have a script ([url removed, login to view]) which creates an options box on my site which allows visitors to show their choice of all deals, only favorite deals, or to view deals 25/page. The problem is, the options box no longer shows up on my page, so some linkage must've become broken. I don't need anything changed in the script itself, I just need someone to figure out why it no longer shows up and to fix whatever's broken.
