Order Form Calculation with Sliders

Hi,

I have an order form that needs to use javascript or php for calculations. Basically calculating selected options and add them up here is an example what I mean;

A client visits the order form, selects couple options, each options he selects or deselects get added and/or deducted, also each selected option should show up under the "order details" window. Few options within the order form are with sliders, so sliding a bar over an option will mean the option is selected. I would prefer jQuery for the sliders. Also, after the name, address, order options, are filled out and selected an email is sent to the admin email address and another email confirming the order is placed and receive would need to be sent to the client.

I'm looking for someone dedicated, someone who can be on top of their timeline within their bid, project is on a budget, would prefer someone within the same timezone, Canada or US.

Please provide at least 3 examples of your work, like forms that calculate, slider control using Ajax, etc.. will help with your bid.
