Integrating a jquery script into wordpress page

I have purchased a jquery plugin script ( 4$ ) which is testing download and upload speeds via browser.

I need someone who can integrate this script into a wordpress page with the following customizations.

1) I need exactly same functionality of the demo with "Show Results in a Dialog" clicked.( modal,lightbox etc )
2) I need a form that have different speed test location options choosable by the user.
3) Above customizations will be integrated on a wordpress page. I will provide hosting for speed test script on different locations.
