customisable flash player for shoutcast stream

I will need a flash player for shoutcast stream.
Will need the source code files .fla / .php
the player will have to play an url like that:
[url removed, login to view]
i would like the url of the stream to clearly appear in the source file (to make it easy to change if needed)
i will need the flash player to have the following function:
"stop" "play" "vol+" vol-" (with a slider) sound "on" off"
+ a small field where people can put a small message and that will be send to a specified email adress.
name of the song / artist to be displayed
and equalizer moving with the music.
the local date and hour where the player is in use.
I will design the player later but need it to be very very very easily customisable in flash, all the button will have to be separated graphic that i can change and easily move like i want.
The job is very urgent and will have to be complete within 48hours.
