Excel File To Responsive CSS/HTML page

I would like this attached excel file to be converted into a responsive form for a website. In the excel file you can see where the input fields are and where the output is. This will be used for a website in desktop and a mobile form so it needs to be responsive.
The user will need to be able to input his or her name and email at the top of this form. This is where the email will get sent once the form is submitted.
There needs to be three buttons on the form.
- Calculate which will calculate the form
- Reset which will reset all the data entered
- Submit which will send an email to me (the company) and an email to the user that filled out the form.
