ASP.NET page to upload PPC Excel spreadsheets into database

Need an ASP.NET Framework 1.0 page to do the following. Must be written in VB.NET.

1.) Allow user to select a given day from calendar control.

2.)Drop down box with the following search engines:

Google, Overture, FindWhat

as choices.

3.) a file upload input box that a user will select a file that will follow the 3 formats in the include.

Once the user posts back with a file then depending on which search engine he selects, I need the file parsed and the following stored procedure called for each row adv_PPCTrafficData_Insert. All database tables and stored procedures are in the include file.

I am using MS SQL Server and the file column to stored procedure parameters mapping is in the include file.
