1. Allow users to login and see their own form and drop-down list.
2. Super administrator backend for me to 'Enable' or 'Disable' user -> Each user will also have an expiry date and if the user is expired, an email notification has to be sent to an 'email' set in the settings page. Also able to add, delete and edit users details such as username, passwords, email, name etc.
3. Each user are allowed to have their own settings, i.e. list of dropdown items, Sent-From Email & name, and designated email, username & password.
4. User will be able to see their own expiry date upon login.
5. Mobile display as alternative display with their customers view
6. Each form must have a simple/standard captcha to verify before submission. This is to avoid spamming which is more than likely to surface. (A MUST)
