I need an app like this one here for my customers.

The app will keep a record of all stamps at various locations.
The app will also need a android based app to instal at tablets and will be placed at restaurants all over my city. The restaurant owner should be able to create a loyalty program from the tablet, and it should generate a QR code so people can scan it when they go shop at the restaurant.
So i need...
1 Android App for restaurant to create program and display QR code.
1 Android App for cell phones to scan and save stamps and cards
1 iPhone App for cell phones to scan and save stamps and cards
1 Blackberry App for cell phones to scan and save stamps and cards
Also when you sign up for our program we want people to enter their name, email and phone number to register
