Open popup child window and fade parent

Sorry for those that bid on last project however I placed it in the wrong budget category. I am needing a script that will open a popup (child) window from a <a href> tag. It needs to fade the background page (parent page), removing focus and allowing a child .php page to be called within it. This .php page will be a form which when submitted needs to be able to load a thanks for submitting form page within the same window. The popup needs to be smooth, clean and stylish. It must be able to be closed from another <a href> tag within the child window (close on click). Preferably the script will use jquery and be light weight however lets see what you have if you have any alternatives that may meet my requirements.
