Simple editing of javascript / re-coding

I have a simple form with javascript making some text appear in a textarea when checkboxes, and dropdown selection are chosen.
My problem is a chained selection I cannot seem to get to work properly.
In short: I am looking for someone with javascript experience to edit/re-code to make it work properly.
Attached file with script that works as intended but does not have chained selection.
