Create a google maps page with user input

Create a one page application using google maps api with javascript.
This page will have two "divs". One to display the map, and the other one have some fields to submit user input.
First the user will click one a certain location, this will create a marker on the map, but for the marker to be registered in the database, the user must complete info in the fields of the other "div" and click submit.
Also, when the user opens the page, it must load the already existing markers that have been registered in the database.
Markers will have different icons depending on the submitted info.
I need someone who has an experience working with google maps api, and who can finish the project in a very short time.
