I run a small airfield with a couple of thousand aircraft movements each year. I'm in the process of creating a new html website using Adobe Muse but the key element is a form which pilots use to tell us that they're coming or going. All the detail is in the pdf I've attached.

There is the potential for this to lead onto some form of airfield management app. For now though I just need a clever form...
