Combining and filtering cells

Hi. In short, I need to see what COMBINATION of procedures are being paid by difference insurance companies. The problem is that each procedure code is in a different row, and you also just can't filter for the same person or code because the two or three codes being assessed must be from the same day. So you can have the person's name or codes come up 5 or 6 times, but on different dates. What I've done so far was filter out the paid column by using an IF statement (if > 0 paid, if 0 not paid). I then created a pivot table where I separated the procedures codes into column of paid and not paid and filtered the person to "count if" names and created a separate row to filter out greater than 2 procedures because excel doesn't let you filter a pivot table and I need to see combination of procedures, not just each individual. However, this isn't entirely accurate because the count if could have been on different dates. Also, I can't then compare to the insurances being paid to have meaningful easy data as data sets I will be working with range from 100k-200k rows. I need help with the set up to be able to apply this to different data.
