Simple PCB design for a completed circuit schematic

I have the circuit drawn up and have already built and tested it on a breadboard. I drew up the circuit on [login to view URL] but got lost at the stage of trying to design the PCB with tracing leads, copper fill and backplanes etc.
The circuit only has 2 transistors, a resistor and a diode so I wouldn't think it would be too hard for someone who is knowledgeable in this area.
What I need now is the PCB design that I can email or take to a SMT shop to have some prototypes printed up for testing.

The 2 connection to lead to the 2 probes are shown as solder pads but I'm not sure that's the way to connect the finished chip to wires, any method of secure connection will work. I'd imagine this chip finished would have 6 connectors of some type (they could all be solder pads if that's whats required), 2 for power, 2 for the motor and 2 for the probes.
Ideally final chip size would be 0.5x0.5" but could be as big as 0.75x2". Please let me know if this is not possible for some reason.
