We receive graphics files from our customers we build yellow page ads from. We need to require our customers to fill some kind of form that would ask a few basic questions about them (Name, phone, acct#, etc.) and attach this data to the email they are sending us. Currently we are useing the AltN MDaemon mail server and would like a solution integrated with it but if this is something we would have to host on a seperate server it would work too.
## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Installation package that will install the software (in ready-to-run condition) on the platform(s) specified in this bid request. 3) Complete ownership and distribution copyrights to all work purchased.
