Add a callout to left column in Magento

We want an image in our left callout on our Magento store, couldn't figure out myself how to fix it.
Can someone please make this? Look at the image in the attachment.
Be aware! Website is responsive, image must be visible when:
@media only screen and (min-width: 768px) and (max-width: 959px)
And when site is bigger than > 959 px
You can put it on the development website, please tell me exactly what files and what lines you've changed, we will put it on the live website ourselves.
