Convert PDF to be an online fillable form

I need the (2) attached PDF's converted and setup so that someone could complete the form online with all the fields, and then hit a submit button which would send the file to an email.
Must look like they do now, but be able to tab through, complete the required info and submit. I am hosting these on a wordpress website, where they will click on the file to edit.
Need done reasonably quickly. Available on Skype for questions once project is awarded.
