Business card plus little stickers

I'd like a business card-like design based on the current look and feel of our website mixed with a typical credit card (see attached). The card should have business info like a business card but also serve as a sticker sheet.
The stickers will be printed on clear plastic vinyl and the idea is you can put them on the top of your credit cards and at a glance know which credit card to pull out of your wallet for your specific purchase (e.g. if you're at the gas station use your gas credit card, if you're at the grocery store use your groceries credit card).
The example attached is just to get an idea, it's a messy ugly example but you're the expert at making things look nice :)
