I have a directory of doctors I need transcribed into an Excel spreadsheet for import into a CRM application. There are approximately 170 pages in the catalog with between 50-60 dentists per page.

I have a directory of doctors I need transcribed into an Excel spreadsheet for import into a CRM application. There are approximately 170 pages in the catalog with between 50-60 entries per page. All of the entries are from the U.S., so those addresses would be in the normal American format.
I can provide the directory in PDF format, scanned at high resolution (600 dpi). I will also provide a full set of instructions for how the data is to be entered, as well as a small spreadsheet example of how the data should look upon entry.
The job must be within 10 days of acceptance of the work, and I would pay a fixed price of $150 for the job, if completed successfully. I would place the entire amount in escrow through vWorker. If vWorker's escrow service will allow, I will release $30 upon delivery of an error-free sample (15 pages worth of data from the overall work.) I would then release more from the escrow as more of the work is delivered to me. However, I will hold at least $50 of the total amount back until the work is completed to specifications, which would be refunded to me if the job is not completed the job on time or the work is not delivered relatively error-free or as promised. These terms are non-negotiable.
