Integrate a Responsive Wordpress theme with a personalized .PSD design

Main goals:
1. To develop a complete and operative/functional responsive web site ready to use for end-users. The website works with database and the data validation need to be exhaustive.
2. I have an uncompleted .PSD and I need you complete/propose the full flow of end-user data acquisition in the website in order to design some more screens in the .PSD to be converted to HTML5/CSS3.
3. To integrate my own design and data acquisition flow completed by you, with all operative and ready-to-use features/functionalities of the licensed theme bought (I will provide privately)
4. To upload all operative/functional website in our hosting service and test it
5. The time-frame for developing the project is 2-3 days
I would like to have a detailed list of activities, tasks, efforts, milestones, costs and deliverables for each task or set of tasks.
In additon, please provide me with:
1. The time dedicated to this project. Example: 8 hours/day; 12 hours/day, etc
2. The opening/working hours in your local and CET Time.
