Create PDF File using FPDF

I need a way for users to create a PDF file of data now displayed via an HTML form. The form now pulls data from a MySQL database in displaying a scoresheet. Given webhost limitations, the PDF file must be created via FPDF ([url removed, login to view]). The PDF file must also closely mimic the original paper scoresheet that the HTML page is based upon. The original paper scoresheet is two legal pages in length, and will be given to the winning bidder.

## Deliverables

1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.

2) Installation package that will install the software (in ready-to-run condition) on the platform(s) specified in this bid request.

3) Exclusive and complete copyrights to all work purchased. (No GPL, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site).
