Customized List CheckBox control

Need a customized list checkbox control. See attached snapshot. The control has to implement UserControl, IScriptControl interfaces. It has to be done from scratch, NO DEPENDENT controls are allowed to be used. On server side (ASP.NET) it has to have Title getter/setter and List<FilterItem> Items getter/setter. FilterItem class is provided below:

On client side (Javascript) it has to handle checking/unchecking checkbox as well as control collapse/expand. So these should not postback. See the attached image to uinderstand.
Also when control is generated (Render(...)) into html markup - no <table> tag is allowed, pls use <div> and CSS styling to get the control.
