Update Wordpress Plugin - Disability Rate Display

Previously, you helped me with a Wordpress plugin that would look up data and instantly display disability insurance rates sorted in order. This was Freelancer project #21889067.

I would like your help making some changes to this work:
1) Will you add an input field called “Package”? the drop-down items the user can select should be “Essential”, “Moderate”, and “Robust”. There should be a new column in uploaded table with one of these words.
a. Example: sample rows in the uploaded data table will be as follows:
i. Male, 26, Non-Tobacco, Allergy & Immunology, *Essential, … 123, 456, 789
ii. Male, 26, Non-Tobacco, Allergy & Immunology, *Moderate, … 234, 567, 890
iii. Male, 26, Non-Tobacco, Allergy & Immunology, *Robust, … 345, 678, 912
2) To condense the display results, can you remove the display column “Sr. No.”. Please leave the data sorted from smallest to largest, but please hide the “Sr. No.” column from the displayed results.
3) On the displayed results, can you add a column? The column should be to the right of the “proposal” column. The column title/name should be “Apply”. The data will be an image linked to a url.
4) After the user clicks “CALCULATE QUOTE”, users seem to be confused. We would like to simplify their experience by showing EITHER the current input fields (First Name, Last Name, Email, Cell Number, Gender, Age, Smoking Status, Specialty) OR the sorted results.
a. When the input fields are displayed, please make sure the results are NOT displayed.
b. After the “CALCULATE QUOTE” button is clicked and results are displayed, please hide the input fields (First Name, Last Name, Email, Cell Number, Gender, Age, Smoking Status, Specialty). In place of the input fields, there should be the text “Your quotes are displayed below”. There should also be a button that says “Get a New Quote”.
c. The “Get a New Quote” button should reset the plugin to the original input screen and hide the results.
5) I would like to keep this installed on [login to view URL] and also add it to [login to view URL] Please make sure the plug in can exist on both pages.
a. On [login to view URL], I only want to display 6 results. I do not want to include the “UAB exclusive” data column in the uploaded data table.
I would like to have this functional by February 5th if that is convenient for you.
Based on your prior work, and these relatively minor upgrades, I’ve budgeted $125 for this project.
