POC to demonstrate Website for Data collection

Develop a proof of concept UI for a data collection portal. Need to demonstrate how data entry screens will look like and flow when entering data. Data will be collected for an entity that will have several objects associated with it. Type of data for an object will be like time they were commissioned in, object was serviced, object type, object manufacturer, object specifications. Right now the work is restricted to developing a POC only and not the actual back-end part. This POC must demonstrate both adding, modifying and removing an entity and it's object's information.
