Hello, My suggestion would be to create a custom template for the popup so you can manage the page from within Wordpress, and to use the mailchimp include codes that you get from their website or to use gravity forms to handle the form. Linking to another page after submit will be an easy task. I am 3 year experienced front/backend developer for web and have done multiple popups and worked with mailchimp a lot.
