Need a form script created

I have a paper form which I am filling out manually. This form needs to be digitalized. There are about 30 fields that need to be filled out. It's basically a question followed by a textual answer. Here's what I'm looking for: 1) Homepage that has a simple header that says "What form would you like to start?" Below that will be text with the different forms. I am hoping to have several forms created in the future, but this project is for the homepage and 1 form only. 2) When you click on the form, it will take you to another page where it asks for header information to go on the top of the form. These will be 6 questions that will make up the header of the form. After filling out the header information, you will be begin answering the questions. Each question should be on it's own page. At the top, it should say Question 4 out of 30, or whatever question you're currently on. When you're done filling out all of the questions, it should display the header, questions and answers on a printable spreadsheet. I have an example of how the form should look when printed, I will share that in private. You should also have the option to save the filled out form in an archive. The archive should display every form I've filled out, and categorize them by form title. The creation date should also be stored, with an option to delete the form. This should be done using SQL. The code needs to be well commented so that I can change questions as needed. I also need to ensure it is responsive, we will be using Ipads and mobile phone to fill out several of the forms. Looking forward to having the same person create several other forms as well, in the future.
