I need a debugging help and a small fix issue for my php web application

I have a simple php web application that i am making. Currently i am experiencing a debugging issue and i dont know where the error lies and how to fix it. The error is likely to be in my AJAX Request, I want you to fix that for me. I want you to capture data from a textbox using JavaScript and send this captured data using an AJAX Request to an SQL Query to retrieve more data using that captured data as where condition in the SQL Query. Note my Application is build using php MVC Architecture and i am using Javascript, JQuery, AJAX and SQL.
