Hire a Website Designer

I need some changes to an existing website. My site is done in PHP it's basically sending text messages to users I will need some help testing the site and fixing small issues to get it up and running I will also need someone to add something to my existing site... Survey I want to be able to create a surveys add questions and answers (I can choose how I want them to answer it for example adding multi choice, single, summary, text etc) after each survey I can also add a redirect link that I want users to be directed too after completing survey I should be able to hyperlink text within survey as well these surveys will be attached to site and I could add a price that I want paid out for each survey The surveys will stand alone on their own page (so I can send links to surveys) I will be able to review the responses submitted under admin area
