I need a great php programmer, My form on my website is not send me the contact info to my email address, it used to work but i have no idea why it stopped, it will be an easy fix for someone who knows what there doing.

Template installed with mailchimp integration
backend admin for template and mailchimp configuration
subscription and auto responder are working from mail chip so no issue with the api
so when someone fills the lead in i want it to email me the information. this used to work but has stopped for some reason. Only people who can fix this quickly apply. I am only willing to pay $30US to fix thanks.
