Just flicking this out to a few people... I need some coding done on an existing website. Unfortunately it needs to be completed by the end of the week so it can be tested over the weekend then sent out on Monday. I had two programmers previously who both failed to do anything (out of their league), hence now the urgency.

So the pages are already designed, they just don't work yet. You can use whatever form script you want but the 3 forms fields (first name, last name, email) need to be populated on
1. the thanks page
2. an email sent to the customer, which will look the same as the thanks page
3. a pdf file, again it will look similar to the thanks page
I also need some way of not allowing repeated entries, as it's supposed to be one per user. So I'm thinking the following:
1. We have each email address save into a database, then when submitted it checks this database. If it's one that's already in there, it won't accept it again and redirects to a different page. which I can build.
Note the website is built in Joomla 1.5 (php), if that's of any help. It currently uses BreezingForms addon, which can do some of this stuff already if you're familiar with it but if you want to use your own scripts that's fine.
I may need to look into a Joomla plugin to allow php parsed onto Joomla articles, as I'm assuming this will be needed.
Let me know if this is possible by the end of the week, and a price? Perhaps quote the pdf file separately, as that's probably of less importance than the other requirements above.
