Create Form w/ pdf upload

I need an area where members can activate their affiliate account. - create affiliate table in existing db, conneced by user ID Fields: - Full name - Business name - Country (select from drop down) - State - City - Address - Zipcode - Phone number - Paypal Email - This field already exist in db, but I need the form to add it to the existing field that is tied to other scripts --- If country is USA, and only USA, then One of these fields is required to submit - TIN (Tax Identification Number) - EIN (Employee Identification Number) - Add check box for AGREE TO TERMS, etc - Add field to table for ACTIVE AFFILIATE - Y, N --- If country is USA, and only USA, then one of these fields is required to submit - W9 SUBMITTED - W, Y, N, /NA - DATE W9 submitted ----------------------------------------------- LOGIC- - Once user fills first form and checks they agree to terms, etc. They will become ACTIVE affiliate, so check Y in ACTIVE AFFILIATE field to activate - If country is USA and only USA, then w9 is required, so check W (waiting) in W9 SUBMITTED FIELD at same time you check Y in ACTIVE AFFILIATE - If members country is USA, and only USA, then show download link for W-9 PDF AND show upload link for member to upload the completed form once complete. - After affiliate has become active, they ave 30 days to submit the W-9 - If country is USA and only USA, and w9 is required, then check Y in W9 SUBMITTED FIELD after w9 is submitted. - Add Date form submitted to DATE SUBMITTED field - If a w9 is required, and NOT submitted within 30 days of activating affiliate account, then check N in W9 submitted field - Deactivate them after 30 days if not submitted, until w9 form is submitted - if w9 is NOT required based on members country, then check NA (not applicable) in w9 SUBMITTED FIELD and they will stay Active
