Add a basic screen to existing PhoneGap/Cordova app with PHP/MySQL backend

We have an existing PhoneGap/Cordova app to which we would like to add a feature allowing a user to create a "question" (text area limited to x characters) with 3 options (also text fields), with 3 other fields (probably dropdown or radio) for category and visibility options. The choices entered by the user need only be saved in the database. You would be responsible for 1) creating the screen within the app where the user populates the described fields; 2) adding to the existing API a new endpoint to fetch the options for the screen described above (probably just the list of categories); and 3) adding a new endpoint which receives, validates and saves the data. The frontend itself mostly consists of HTML, JS/TypeScript and CSS/LESS - and (for legacy reasons) is a mashup of various frameworks, but you'll probably only need to concern yourself with Vue.js and possibly jQuery. The backend is mostly PHP (Laravel 4.1) and MySQL. The original programmer will be available to help you find your way around the existing code if necessary. We would strongly prefer to find someone who would be available for future work on other projects.
