DIALPLAN Asterisk with MySQL

Hello, I need 2 dialplans custom for Asterisk. 1th - Satisfaction Survey The customers will be redirect to a IVR (Example: IVR 200). The IVR will say a prompt voice: "Welcome, please, avaluate we Services". The IVR will say a prompt voice: "How you evaluate the time of service, 1 is very good and 5 is too poor". >>The AGI will collect the 1, 2, 3, 4 or 5 digit of customers and send to MySQL table "TimeService".<< He will send to another prompt voice: "How you evaluate the total service, 1 is very good, and 5 is too poor". >>The AGI will collect the 1, 2, 3, 4 or 5 digit of customers and send to MySQL table "Totalevaluation".<< The IVR will say a prompt voice: "Thanks for you help. Good Bye". The Mysql structure need be: phone - Date - hour - TimeService - Total Evaluation 2nd - IVR Protect The customers will be redirect to a IVR (Example: IVR 201). The IVR will say a prompt voice: "Welcome a Service Center". The IVR will say a prompt voice: "Digite the Account Number". >>The AGI will check the account number in MYSQL, IF Account number don't exist, will say: "Customers don't find, and return to prompt. If account number exist, will say: Customers Find, wait a moment.<< >> The AGI will check, if fulltime are yes, will redirect to QUEUE. If comercialtime if yes and dont is comercial Hours (timecondition), will be prompt ("You account don't have support in this time, please, call in Comercial Hours"). If custom are yes, will call to digit a password. If password are OK, goto queue, else, try 3 times and later say prompt ("Incorrect Password, good bye"). The Mysql structure need be: Account Number - Customer Name - fulltime - comercialtime - custom - password Fulltime, comercialtime and custom are yes or no options I preffer the AGI direct in DialPlan, without PHP. Or a PHP structure will me modify when I need.
