Create contact forms for wordpress site

I am looking to hire immediately. This work must be completed today (within the next 8 hours) My budget is $30, If you cannot accept this budget then please do not bid. Current site: [login to view URL] I need 3 contact forms created: Modal Form - This is the popup form that we want to be usable from many buttons throughout the site (just like on the current site). It has three fields: Name, Company, Email (all required) Quick Form - This is meant to go on the Company Page. It will go in the blank space on the right, just under the header. It has two fields: Name, Email (both required) Contact Form - This is the full form that will go on the actual Contact Us page. On the lefthand side. It has multiple fields: Name (required), Company, Email (required), Phone, Message (required) We want the forms to keep the look and feel of the site (which the partially do already if you just make a form and drop it into a page using the visual editor. But it doesn't look quite right and doesn't have the popup functionality. Also, one thing that we have on our current site (which uses Gravity Forms) is that the email we get from a form submission doesn't just have the the info from the form fields, it also tells us what page the contact form was sent from. Since the modal form is the same everywhere, it's helpful for us to know if someone decided to contact us from the homepage or the product page. This id super simple with Gravity Forms, but I don't know much about Contact Form 7. I am looking to hire immediately.
