Create basic webpage to allow user to create custom video like Facebook Year in Review videos

I want to create a site that allows user to create short, customized videos similar to the facebook year in review videos. You can see a sample here: [login to view URL] The videos will be provided by me, and the user should be able to add their own text that will display in the video, and upload images to display in the video. The video should be able to play on web and on mobile (including ios, so flash may be a problem). The first step for this project for you to bid on, should be a very simple working demo of this. This will include a form page, where user enters 3 text boxes and uploads 2 images. Then after the user submits the form, the next page plays the video with the 3 text and 2 images in the video, just like the facebook video example.
