We need an online form to be created on our wordpress site , I have attached the pdf Ver of the form ( All images are available) .

Working :

1. A link will be provided to our dealers ( [login to view URL]<link>

2. They will have a basic authentication to access the order form

3. They will fill in the required fields of the form , which will have drop down boxes for the Qty they order , prices are fixed for all .

4. They will submit the form

5. A confirmation email will come to us and to them of the confirmed order

As a security measure if the ip is not from NZ then it should not allow any login. I dont want any robots to crash the site .

Please be specific when quoting . Please ask questions if you are not sure of anything .
