Change php and html from radio buttons to drop down boxs.

I have included the php file and a screen shot of the current layout. I am needing the two areas changed from radio buttons to drop down boxes. The text in blue will be in the drop down box for the first items and when one is selected there is info that will display on the page. I did not include the function for that so I will have to test that on my end. The second radio button will work the same way but this time you will also need to create a java script fuction to display the rest of the information once one of the two choices is picked because there is too much info in the radio box text right now. I will not pay more than 20.00 for this project because it is fairly easy. It is a test project for me too find a good coder for future work. PROJECT MUST BE COMPLETE WITHIN 6 HOURS OF ME AWARDING IT.
